3. Print, sign and fax to the number listed on the form

Once the referral is received, the Sleep Center will call the patient to make an appointment.

The Sleep Center is located on the tenth floor of The Â鶹¹ú²ú in a pleasant, quiet atmosphere. To make your stay comfortable, we prepare all paperwork ahead of your schedule.

All tests are done after obtaining approval from your insurance company. Wait times (if any) are short and we offer a secure, free, parking facility.
